Certificate of Immunity

Hello New Students!

The Health Center welcomes you to the Saint Xavier University community. As a student at SXU, you have complete access to the services offered at the Health Center. Our nurse practitioners and other staff members are available for minor illness/injury care, chronic illness management, physicals, immunizations and laboratory work.

Our staff supports the academic mission of student learning by assisting you in leading a healthy life through our health promotion, risk reduction and health education efforts. In order for us to best serve you, though, it is necessary for us to receive your Certificate of Immunity form.

Please know that all SXU students are required by Illinois law to have a completed immunization record on file with the Health Center.

The State of Illinois law requires that all students born after January 1, 1957, show proof of two measles (rubeola) vaccinations given after the student's first birthday, one mumps and one rubella vaccinations. The State of Illinois law also requires a current (within the last 10 years) Tetanus-Diphtheria immunization. If your birth date is before January 1, 1957, (a) it will not be necessary to show immunization dates or proof of immunity, except for the tetanus shot within the last 10 years (b) include a copy of your birth certificate or valid driver's license.

The Certificate of Immunity form is mailed to each student's permanent home address and may also be downloaded (PDF).

This form must be signed by your health care provider with verification of dates of immunization or you may attach a copy of your immunization record. Completed forms should be returned to the Health Center. Forms may be mailed, dropped off or faxed to 773-298-3906.
