As a patient of Saint Xavier University Health Center, you have choices, rights and responsibilities.

You Have The Right:

  1. To receive considerate and respectful care.
  2. To know the names and professional status of people involved in your care.
  3. To expect that your personal privacy will be respected by all staff.
  4. To expect that your medical records will be kept confidential and will be released only with your (or, as appropriate, your parents/legal guardian’s) written consent except as otherwise required by law.
  5. To receive accurate information about your health related concerns.
  6. To know the effectiveness, possible side effects and problems of all forms of treatment.
  7. To participate in choosing a form of treatment.
  8. To receive education and counseling.
  9. To review records concerning your care and treatment.
  10. To know information about services and related costs.
  11. To express complaints about the Health Center or the care received.
  12. To refuse treatment and know the effects of such refusal.

You Have The Responsibility:

  1. To seek medical attention promptly.
  2. To be honest about your medical history.
  3. To ask about anything you do not understand.
  4. To follow health advice and medical instruction.
  5. To report any significant changes in symptoms or failure to improve.
  6. To respect health center policies.
  7. To keep scheduled appointments or cancel in advance.
  8. To seek non-emergency care during regular Center hours.
  9. To provide useful feedback about services and policies.
  10. To pay any charges billed to you.
  11. To treat staff with courtesy and respect.
