Certificate of Immunity
Hello New Students!
The Health Center welcomes you to the Saint Xavier University community. As a student at SXU, you have complete access to the services offered at the Health Center. Our nurse practitioners and other staff members are available for minor illness/injury care, chronic illness management, physicals, immunizations and laboratory work.
Our staff supports the academic mission of student learning by assisting you in leading a healthy life through our health promotion, risk reduction and health education efforts. In order for us to best serve you, it is necessary for us to receive your Certificate of Immunity form.
Please know that all SXU students are required by Illinois law to have a completed immunization record on file with the Health Center.
The State of Illinois law requires that all students born after January 1, 1957, show proof of two measles (rubeola), two mumps, and two rubella vaccinations given after the student's first birthday. The State of Illinois law also requires proof of three Tetanus-Diphtheria immunizations, one must be a Tdap (pertussis protection), and one must have been given in the last 10 years. Additionally, all students under the age of 22 must show proof of a Meningitis A vaccinations after their 16th birthday.
It is preferred that you submit your full immunization record, OR you can download the 2016-17 Certificate of Immunity form (PDF) for your health care provider to complete. Completed forms should be returned to the Health Center. Forms may be mailed, dropped off or faxed to 773-298-3906.
