Financial Aid Forms

Verification Forms

If your Free Application for Federal Student Aid (FAFSA) was selected for review in a process called verification, Saint Xavier University must gather additional information about you and your parents (or spouse), as required by the U.S. Department of Education (34 CFR, Part 668), in order to process your financial aid. Please submit all required documents as soon as possible to prevent delays.

2017-2018 Verification Forms

Please note: If selected for 2017-2018 verification, you do NOT need to complete all of the forms below. The necessary documentation varies by student and is detailed in the email (sent to your SXU email account) by the Office of Financial Aid and listed in your Required Documents in mySXU Self-Service. Please refer to Self-Service or your email, or contact the Office of Financial Aid, to eliminate the possibility of completing more documentation than required.

Income Verification:

If the Office of Financial Aid is requesting Income Verification for you or your parents (or spouse), the most efficient way to satisfy this requirement is to make a revision to your FAFSA, transferring in your tax information utilizing the IRS Data Retrieval Tool.

Unable to Link or choosing not to? Alternatively, you may obtain and submit a 2015 IRS Tax Return Transcript. Please note: IRS Tax Return Transcripts are paper documents mailed to you from the IRS. This process may take two weeks or more.

Verification of 2015 Income Tax Nonfiling:

If the Office of Financial Aid is requesting Verification of Nonfiling for you or your parents (or spouse), you must provide confirmation of nonfiling from the IRS to confirm you did not file a 2015 Federal Tax Return. A confirmation of nonfiling can be obtained using IRS Form 4506-T and checking box 7.

Download IRS Form 4506-T and submit it to the IRS, then provide the Office of Financial Aid a copy of the resulting IRS letter that is mailed to you.

Appeal Forms

Special Circumstance Appeal Forms can be used by students to report unusual circumstances that may impact their ability to pay for their education at Saint Xavier University. These circumstances can be conditions that have reduced the family's income for the current year (e.g. loss of job; reduction in wages; loss of one-time income) or extraordinary expenses that provide the family with less disposable income (e.g. significant medical expenses; elementary/secondary private school tuition for siblings). 

Dependency Override Forms are available to students who are determined, by the Free Application for Federal Student Aid (FAFSA), to be classified a dependent student, but meet limited conditions to be considered an independent student. 

To obtain Special Circumstance Appeal or Dependency Override Forms, please contact the Office of Financial Aid. A Financial Aid Counselor will assist you in evaluating your eligibility for consideration.
